Thank you for referring your student and family to CREST! It is our hope that our school can fulfill your student’s educational needs.
All referrals must be sent directly to Referral Coordinator, Andrea Holt, aholt@crestcollaborative.org, and the Executive Office to include Bill Bryant, Director of Programs, bbryant@crestcollaborative.org, and Cyndie Femino, cfemino@crestcollaborative.org.
The CREST referral process begins with a records release and referral packet from the sending school District. The referral packet must include a signed IEP.

Extended evaluation form (If applicable)
Current IEP or last signed and proposed IEP
Latest evaluations
Behavior/Incident reports, Behavior Intervention Plan (if applicable)
Suspension records (if applicable)
Once received Director of Programs along with the Executive Director will begin to determine whether placement in one of the CREST programs is appropriate for the student. Upon determination, CREST will reach out to both District and Parent/Guardian to schedule tours.
